Poster Video Presentation Submission

I make the following representations and grant the following rights to SCAI - The Society for Cardiovascular Angiography and Interventions (SCAI):

1. I warrant that the presentation and all accompanying written, pictorial, and video material (collectively the "Work") is original with me or that I have obtained the necessary rights to make this release, and that its presentation and publication will not infringe the rights of others.
2. I grant to SCAI the perpetual, royalty-free, nonexclusive right to the use, transfer, adapt, and sell the Work at our annual conference and world-wide, including at our other meetings and on our website and by other electronic and other means, and in any other place, medium, format, charge, or language as determined by SCAI.
3. I grant to SCAI the right to use my name, likeness, photograph, and biography in connection with the Work in whatever form, fashion, or medium, including print, electronic, or otherwise for as long as SCAI wishes.
4. I consent to the participation in interviews, the use of quotes, and the taking of photographs, movies or video tapes by SCAI. I also grant SCAI the right to edit, use, and reuse said material for any purposes and activities permitted within its mission, however distributed or sold, including use in print, internet, OnDemand, licensing and enduring content, and all other forms of media. I also hereby release SCAI and its agents and employees from all claims, demands, and liabilities whatsoever in connection with the above. I hereby warrant and represent that I am at least 18 years of age and have the full right to contract in my own name with respect to the matters stated above.
5. This consent provides SCAI and any of its vendors, including Capital Reach, LLC, with my consent, waiver, and release of liability. By selecting “I confirm that I am the presenting author and I agree to the terms,” I acknowledge that I have read and understood all of the terms of this consent and agree to the playback of my video as submitted to both the audience at SCAI 2020 and for release to the SCAI webcast website either live or on-demand playback.
